Are you supposed to wear anything under scrubs?

Excuse me, I was wondering if it's necessary to wear anything underneath scrubs when working in a medical setting? I've heard different opinions on this topic and I'm curious about the standard practice. Is it expected to wear a specific type of clothing, such as a base layer or underwear, or is it perfectly acceptable to wear scrubs directly on the skin? Your insights would be greatly appreciated.
